When performed correctly for facial massage, Gua Sha should not hurt and should not cause bruising. You should use a light-to-medium pressure—enough to feel a deep massage but not so much that you cause pain or significant lasting redness. For the body, deeper traditional gua sha can intentionally cause temporary reddish spots (petechiae), but this is not the goal for facial gua sha.
